How we assess PCOS

PCOS is not one condition with one cause. Two people with the same diagnosis can have very different drivers — one primarily insulin-driven, another with adrenal androgen excess, another where the picture is confused by thyroid dysfunction. Treating them identically is why so many people find that standard advice does not work for them.

So we test before we treat. Depending on your presentation that may include total and free testosterone, SHBG, DHEA-S, LH and FSH, AMH, prolactin, a full thyroid and metabolic panel, fasting insulin and A1C. Insulin resistance is common in PCOS and routinely goes unmeasured.
