Step one: testing, so we are not guessing

We start with comprehensive bloodwork. Depending on your symptoms and history, that commonly includes a full thyroid panel, ferritin, B12, vitamin D, fasting insulin and A1C, and sex hormones including estradiol, progesterone, FSH and LH. Often more than that, depending on where your history points.

Whether we add DUTCH or saliva hormone testing is a decision that comes out of the conversation, not a box we tick by default. It earns its place when your symptoms do not line up with what serum results show, or when your cortisol rhythm and the way you metabolise estrogen genuinely matter to the plan. If a test will not change what we do next, we do not run it.

If your cycles have become heavier or less predictable, we look carefully at iron. Heavy perimenopausal bleeding is one of the most common reasons ferritin drops quietly over months, and low ferritin produces the same exhaustion, brain fog and hair shedding that usually gets attributed to hormones alone. When stores are depleted enough that oral iron cannot catch up, an iron infusion may be appropriate.

We look at nutrient status directly as well. Several deficiencies become more common during perimenopause, and they are frequently mistaken for the hormonal transition itself.

Step two: a plan built from your results

Once we can see what is actually happening, we build the protocol around it. Depending on your findings, that may include:

  • Nutrition strategy aimed at blood sugar stability, inflammation and estrogen metabolism, rather than generic advice.
  • Supplement protocols chosen against your results — adaptogens, B vitamins, magnesium, iron, vitamin D and herbal support where both the evidence and your bloodwork justify it.

Step three: follow-up and adjustment

Your first appointment is 75 minutes. That length is deliberate. Perimenopause rarely arrives as one clean complaint, and a short visit cannot separate thyroid from iron from cortisol from the hormonal transition itself.

We follow up about a month after your plan begins, and after that as needed. Perimenopause is not a fixed state — what your body needs at 41 is not what it needs at 46, and the plan is expected to change with it.
